Collects information regarding services and needs of elderly Native Americans and ensures they receive available services. Maintains daily log of service requests; makes home visits to provide general services' assistance to the elderly; creates and maintains case files; and provides limited transportation, and participates in case planning.

Job Duties:

Collects information from various sources regarding service needs of elderly Native Americans residing within the jurisdictional boundaries of the Cherokee Nation. Electronically transmits (fax) service requests to the assigned advocate or manager for determination of eligibility. Maintains daily log of service requests and submits to designated individual for statistical purposes. Makes home visits to provide general services' assistance to the elderly. Creates and maintains case files containing consumer's services request and status of service provision. Provides limited transportation for elderly consumers when there are no available resources. Assists elderly consumers in completing applications for services, i.,e., Social Security Administration forms, Veterans affairs, etc. Maintains resource materials such as pamphlets and brochures as relates to the department. Works with community organizations and volunteers to identify elders in need of services and helps identify possible gaps in services received. Provides information services to community groups, church groups, and volunteer groups as requested and scheduled. Attends scheduled trainings regarding general services to the elderly. Participates in case planning as requested. Interacts with all other services offered by the Cherokee Nation included but not limited to federal programs. Other duties may be assigned.


No supervisory responsibilities



Associate's degree from 2-year college or technical school; or two years (60 credit hours) coursework from 4-year college or university in Social Work or related field; or three years related experience; or equivalent combination of education and experience.


Two years of related experience is required.


To perform this job successfully, an individual should have knowledge of Word Processing software.


Must possess a valid driver's license with a driving history verified through a motor vehicle report that meets requirements for Cherokee Nation underwriting rating.


Must have working knowledge of office machines. Musk know how to email and fax. Must have good people skills.


Bilingual skills (Cherokee) would be an asset in this position.
