General Summary:
Provision of home-based casework services for families involved with DCS/Probation. Home-based casework for pre-adoption and post-adoption services for adoptive families at risk or in crisis. Home-based Caseworker Services (HCS) provides a combination of services to the families. Case management may include but not limited to, supervised visitation, coordination of services, education and training, advocacy, referrals and follow-up, life skills training, and facilitator of transportation

Minimum Education Required

Bachelor’s Degree required; mental health field of study preferred.

Minimum Experience Required

Experience in mental health or social services preferred.

Essential Functions

·

Adheres to and supports the Division of Mental Health Policies and Procedures and established guidelines.

·

Conducts strength-based assessment skills effectively, incorporating communication, listening and interviewing techniques.

·

Coordinates with employment specialist feasibility of vocational program in conjunction with client’s benefits.

·

Demonstrates the safe and effective use of all equipment used in the employee’s activities.

·

Maintains a safe environment and adheres to infection control standards.

·

Identifies personal continuing educational needs annually and actively participates in educational opportunities offered within CHRH.

·

Develops and reviews treatment plan for assigned clients according to hospital policy and procedures.

·

Implement treatment plan, emphasizing community living and problem solving skills relating to the home, school and/or community environment, according to program objectives.

·

Leads and/or participates and effectively communicates with Multidisciplinary Team in the provision of quality patient care.

Attends Multidisciplinary Team meetings in accordance with predetermined schedule to facilitate patient care.

·

In consultation with Multidisciplinary Team/Supervisor, evaluates client’s needs for treatment programs and makes appropriate referrals in accordance with the client’s treatment plan.

·

Provides appropriate level of needed supervision and case management to all clients.

·

Completes all paper work including all clinical documentation in a timely manner and in accordance with hospital, divisional, and departmental Policies and Procedures.

·

Coordinates contacts between assigned clients and outside community social agencies, (i.e. Social Security, Child and Adult Welfare, family and schools).

·

Develops agendas and facilitates groups to meet client’s treatment needs.

·

Assists clients in locating appropriate residential settings according to program guidelines.

·

Monitors the stability of client living situations conducting regular home visits in accordance with established program guidelines.

·

Transports clients as necessary in accordance with established Policies and Procedures.

·

Assists in moving clients utilizing community resources including Supported Employment.

·

Maintains contact with family of client, when appropriate, to ensure quality and continuity of care in accordance with treatment.

·

Addresses the developmental, bio-psychosocial and functional needs of the client population served.

·

Provide vocational case management services when needed

.

·

Case Managers will work to establish and improve client’s/legal guardian’s autonomy with regard to medication and money management.

·

Attends department staff meetings and/or reads and initials minutes.

·

Maintains the privacy and security of protected health information (PHI), the confidentiality of all information, and conducts all aspects of patient care charting, billing, and all operations within the system in a professional and ethical manner in accordance with Federal, State, and CHRH rules and regulations.

Communications with all departments of CHRH, community, regulatory, and insurance agencies.

Requirements

Must be able to read, write, and speak the English language.

Must possess good communication skills and maintain a professional and business-like manner.

·

Must possess the ability to make independent decisions, display initiative, and be responsible for performing job duties with supervision.

·

Must maintain client confidentiality.

·

Must maintain professional demeanor by not becoming involved in power struggles, rumors, or other controversial area of work.

·

Able to establish and maintain appropriate boundaries in relationships with clients.

·

Maintains cost-effective methods.

·

Within 30 days of employment, must obtain a Chauffeur’s license and must have dependable transportation.

Must be able to speak the English language in an understandable manner.

Must be able to cope with the mental and emotional stress of the position.
