Supervise assigned interdisciplinary clinical team providing support to clinical field staff.
Ensure that the Plan Care is properly administered working with internal staff, referral sources, patients and their caregivers.
Provide excellent customer service to patients and their families, referral sources and internal staff.
Train and educate field staff. Conduct new hire orientations and supervisory visits as needed.
Ensure office and staff compliance with both state & federal guidelines.
Review charting and documentation to ensure proper delivery of care and correct documentation. Create and update reports for Director of Clinical Services and/or Executive Director.
Keep Director of Clinical Services/Executive Director abreast of all clinical updates and issues
Oasis Knowledge to include hands-on experience with review.
Excellent communication skills, both verbal and written.
Upbeat, positive attitude and the ability to work in fast-paced, sometimes high-stress environment.
Demonstrated leadership, problem solving and analytical skills.
Passion for ensuring best care and outcomes for our patients.
Associate or bachelor degree of nursing.
Current RN license in state of agency operation.
Two years of recent home health experience in a Medicare setting.
Minimum of one year management experience in a clinical setting.
Must have valid state driver’s license and insurance.
