Job Description: The OACIS Clinical Coordinator is responsible for assuring that OACIS patient needs are met, assisting the MD's and Advanced Practice Nurses with providing service to inpatients.

As directed by the medical director and program director, the coordinator is the point of contact for patients, referral sources and hospital staff to the OACIS program, assuring that all staff time is maximized. Attends to clinical needs as well as data needs of the program. Minimum Requirements: Graduate of an accredited school of nursing. Active Pennsylvania Nursing License (RN or LPN) Influenza vaccination will be required for all personnel working within LVHN, who have patient contact, unless medically contraindicated or due to a bonafide religious belief or practice.

Preferred Qualifications: Experience as a nurse in an office setting. Palliative Care experience preferred. Board eligible for Hospice and Palliative Care certification. Licensure and Certifications: Active PA RN or LPN license required.
