HealthSouth's hospital based Home Health agency - HealthSouth Therapy at Home - is currently looking for an Occupational Therapist to join our agency. Our agency is located insided the HealthSouth Rehabilitation Hospital of York. Address: 1850 Normandie Drive, York, PA 17408 HealthSouth's Home Health programs provide skilled nursing and therapeutic services, such as physical, occupational or speech therapy, medical social services, and other home medical arrangements.

Our team of Home Health professionals works closely with the patient's physician and family to develop a plan of treatment, tailored to meet the patient's individual healthcare needs. This team approach ensures consistent, coordinated care from professionals who understand the unique needs of homebound patients. The Home Health Occupational Therapist provides occupational therapy treatment and patient care according to hospital, Home Health, state, professional and federal regulations and guidelines. Typically our therapist can expect to spend an average of 2-4 weeks with our patients treating a wide clinical caseload such as: stroke, spinal cord injury, brain injury, various neurological disorders, rheumatoid arthritis, osteoarthritis, amputation, joint replacement, fractures, multiple trauma, cardiac conditions, and pulmonary conditions. We use evidence-based practice guidelines to treat our patients. We utilize our clinical outcomes to asses and improve our treatment interventions and programs. We participate in a national outcomes database, which provides benchmarks and sophisticated outcome reports to help us continually analyze and improve our patient outcomes. The Home Health Therapist also has specialized Home Health trainings, clinical therapy resources available from the Inpatient Rehab Facility, ability to meet with patients in-house and be an integral part of the continuum of care. Helping you become a better therapist is critical to our success - You will be paired with mentors ; attend in-house as well as external educational programs. The treatment team is made up of full time therapists, nurses, social workers and medical staff; a great way to learn beyond your own discipline.
