The staff members cover all units of the hospital seven days a week. The shifts will be 4 days a week for 10 hours a day for full-time employees.
These shifts will be flexed from 7 AM to 10 PM to provide continuity of care.
The staff members provide Mid-Level support to the Ernst Cardiovascular Clinic Monday through Friday for the heart failure patients.
Additional Service-Specific Duties:
Primary management of all CHF patients under the direct supervision of the Medical Director of the CHF service.
Assess and admit if necessary patients who present to the EC with possible or presumed CHF exacerbation.
Daily rounds on the CHF patients coordinating their schedules and patient lists with the HF Nurse Clinicians.
Pre Op teaching for both in-patient and out-patient.
Write daily progress notes detailing patients’ primary diagnosis as well as co-morbidities with complete assessment and plan of care for all active problems.
Be able to order and review indicated labs, diagnostic tests, and medications in order to monitor the patient’s progress and modify an appropriate plan of care.
Communicate with cardiologist attending as well as consulting service physicians to streamline care
Attend regular family meetings to update them on the plan of care as well as formulate a discharge plan – act as physician designee for discharge planning.
Act as primary liaison with continuing care in the discharge planning process for the CHF patients.
Ensure compliance with the CHF indicators to match Core Measures upon discharge, including the discharge Medication Reconciliation.
Review and write discharge AVS for heart failure patients in collaboration with HF nurse clinicians.
In-services for nursing and medical staff will be provided.
Will cross train with the coronary team.
Current certification as NP or PA, valid NP or PA Michigan license required, BLS from the American Heart Association required.
ACLS and CHF experience preferred
