The Case Manager-MSW serves as a leader of the discharge planning process and a valued member of the healthcare team at Novant Health by establishing a personalized relationship with the patient and their chosen support system, collaborating with physicians and other team members to assess, plan, implement, and evaluate a discharge plan that promotes a safe and timely discharge. The Case Manager-MSW takes the lead on complex patient discharges and crisis intervention situations such as abuse, neglect, and family/criminal violence. The Case Manager-MSW utilizes compassion to assist the patient in navigating their health journey. The Case Manager-MSW accepts responsibility, authority and accountability for management and provision of care in accordance with the current policies and procedures. The Case Manager-MSW provides age-appropriate psychosocial assessment, crisis intervention, information/referral, coordination of services, patient advocacy in relation to the provision of direct services.
- Education: Master's degree required.
- Experience: Minimum of two years professional social work experience required. Health care experience preferred.
- License/certification/registration: BLS/Heartsaver as indicated.
- Additional skills required: On call: weekend and holiday coverage; ability to travel between facilities.
