Position Summary:

Reporting to the Chief Clinical Social Worker, the CSW is responsible for providing for the delivery of social services including psychosocial evaluation and treatment, advocacy, teaching, referrals to community services, and assisting as needed with discharge planning. Assists with the development of departmental policies and procedures, and ensures adherence to them.

Salary Range:

Rate Minimum $60,030.00 Maximum $71,743.68


Accurately coordinates education needs for the discharged patient/family

Acts as an effective liaison to medical staff to ensure continuity and congruity of hospital services in accordance with the plan of care

Collaboratively participates in the care of the dying patient, respecting patient choices

Consistently demonstrates an advanced level of clinical expertise and enhanced communication skills

Coordinates functions for various hospital surveys for approval by regulatory agencies

Demonstrates a high level of social work competency and works within acceptable professional and community standards of practice

Develops and maintains quality improvement activities and recommends/implements program changes as appropriate

Develops care plans that are specific to the disease and age of the patient

Effectively collaborates as needed with the patient, family and healthcare team to facilitate appropriate patient transfers

Effectively coordinates multi-disciplinary referrals

Effectively intervenes in instances of abuse, assault or neglect, or when adequate financial or social resources are not available

Facilitates effective and efficient hospital response to physician orders and the acquisition of necessary services

Maintains knowledge of the impact of physical disabilities, chronic, acute and terminal illness on patient and family well being

Provides effective education, direction and guidance to clinical staff

Serves as an effective role model and preceptor to staff

Supports the participation of patients in planning for their care

Willingly assists patients who request to execute a Durable Power of Attorney for health care, or other document of choice

Required Qualifications:

Master's Degree in Social Work

Knowledge of and ability to work within a complex health care system, with strong oral and written communication skills

Skills to interpret and implement hospital, JCAHO, Health Department and Child Abuse Registry standards and regulations

Preferred Qualifications:

Current Clinical Social Work License issued by the Board of Behavioral Sciences

Knowledge of UCI Medical Center policies and procedures
