Be responsibile for implementing, maintaining, and evaluating the Home Base health care model designed to provide patient-centered care including disease management for employees/dependents.

Be accountable to provide support and coaching to Home Base care coordinators (LPN/RN), clinic managers, medical directors, providers, and staff to improve and maintain the Home Base Care Model elements.

Work closely with the Home Base primary care providers, care coordinators, other clinical staff, and support staff as well as the Clinical Design team and be responsible for maintaining the Home Base Model at clinics where already implemented.

Be responsible for partnering and collaborating with Providence Medical Group (PMG) Leadership, including Chief Physician Executive, Chief Executive Physician Services WMR, participating physicians, and clinic leadership to identify and spread best practices in using resources, tools, and processes to support the Home Base Model.

Be responsible for gathering and analyzing data to assess impact of program on clinical outcomes and quality, patient experience, and cost.

Required qualifications for this position include:
Bachelor's degree + four years experience in project management, information technologies, quality improvement, change management, healthcare administration, disease management, or other clinical field.

Demonstrated expertise in performing operational and clinical data analysis and interpretation to guide decision-making for improvement.

Knowledge of project development and management including planning, budgeting, and decision-making.

Valid drivers license, motor vehicle report, reliable transportation and proof of current insurance.

Preferred qualifications for this position include:
Master’s degree in public health, an RN, or other clinical degree/licensure highly preferred

Experience in population-based disease management (PDM)

Experience in employee wellness programs a plus.

Use of evidence-based clinical practice guidelines and disease registries in an outpatient setting

Strong computer skills and experience working with electronic medical records, registries, and access databases.
