Primary Health Care, Inc.(PHC) seeks Board Certified Family Medicine Physicians without OB to provide primary patient care services. PHC is a non-profit community health center dedicated to serving the uninsured and underinsured since 1981. The physician to join our team must be passionate about our mission to provide health care and supportive services for people in Central Iowa to improve their quality of life.
PHC strives to find and successfully treat health conditions before they become serious enough to require hospitalization or emergency treatment, as well as offering continuity of care for acute and chronic problems. The clinics currently have 15 - 18 exam rooms and the physician swill work as a team with other physicians and mid-level providers. PHC offers a competitive benefits and compensation package. Federal and state school loan repayment could be available. Very limited call coverage. Bilingual Spanish is a plus.
Primary patient care services including general medical care, family planning, diagnostic laboratory, HIV testing and counseling, testing for blood levels, and immunizations. Additional diagnostic radiology services and other diagnostic testing is provided by referral. Providers are currently participating in the Patient Center Medical Home, which is a nationwide effort at improving outcomes of patient care.
Obstetrical and Gynecological Care, which includes gynecological care, prenatal care, antepartum fetal assessment, and postpartum care. High-risk care, ultrasound, and labor and delivery services are provided by referral.
Dental Care services include preventive, restorative, emergency and rehabilitative care. Patients who lack adequate financial resources as well as other specially designated groups are given priority to dental care.
Pharmacy Services are offered at the Community Access Pharmacy located at our Outreach Clinic on University Avenue. Patients have access to low cost prescription drugs through the 340B pharmacy program. In addition, assistance is provided to patients in procuring pharmaceutical company’s patient assistance program (PAP) medications at minimal cost.
Other Professional Services are offered at all of the service delivery locations including hearing and vision screening. Occupational, vocational, physical therapy, optometry and podiatry are offered by referral. Nutrition and ophthalmology specialty services are offered via contracted providers.
Mental Health/Substance Services are offered on site at all clinic sites. In addition, the Enhancement Program of the Outreach Project provides substance abuse assessments for homeless populations and makes treatment referrals.
Supportive Services including interpretation/translation services, transportation assistance, and environmental health risk reduction, health education, and nursing home and assisted-living referrals, are available at all locations. Additional supportive services are offered at the Outreach Project locations and include case management, eligibility assistance, outreach, home visits, and prenatal education.
