Under the direction of the Hospice Executive Director performs psychosocial services for Hospice patients and their families. The Social Worker will develop a plan of care through the use of social work practice and theoretical and experiential knowledge for infant, child, adolescent, adult and geriatric patient/family. Performs other duties as assigned.

Job Function

Provides for pre-admission and initial admission visits to patient and families and completes required admission paper work Completes initial and ongoing psychosocial assessment of infant, child, adolescent, adult and geriatric hospice patients/families and documents in patient's clinical record Develops, with other interdisciplinary team members a plan of care for each hospice patient/family based upon the psychosocial assessment and reflecting various patient/family cultural, spiritual educational, developmental and value contexts and documents in patient's clinical record Implements and evaluates plan of care and documents patient/family response to plan of care Coordinates available financial and community resources for hospice patients/families Develops and coordinates discharge plans for patient's who revoke hospice care or are discharged from Prairie Haven Hospice Assists in providing community education regarding Hospice and grief related topics Participates in ongoing quality assessment and performance improvement activities


Bachelor's degree in social work Master's degree in social work preferred Certified as a social worker in State of Nebraska One to two years experience in social work in healthcare setting. Prefer one to two years in a home care and/or a hospice setting
