Position Summary:
To serve as the key physician within a Pod (regional area) of CMG’s healthcare center network for the CustomCare program, and to further the goals of CMG in areas such as:

Quality of care

Customer experience, access, and satisfaction

Advancement of the Patient-centered medical home model

Financial performance

Employee retention and satisfaction

Clinician productivity

Relationships with associated external specialty physicians and healthcare institutions

The physician will be the lead clinician within each Pod’s CustomCare program. This will include providing direct clinical care of patients within the program, and working closely with the Associate Clinician and staff within the program and within and associated primary care practices with the Pod.

The direct clinical work and associate clinician supervisory work will include providing care to patients within these specific CustomCare initiatives:

  • “TLC” chronic disease management program
  • “At the Home” transition of care post-discharge program
  • “Home-based Care” program
The physician will not be the manager of the associate clinicians, but will work closely with them and supply them support from clinical, patient care, and programmatic standpoints.

A working partnership must also be established with the administrative leaders in CMG, including the CMO, other Medical Directors, Clinical Operations, Ancillary Businesses, Population Health and external Specialty Partners and Healthcare Institutions.

The physician will be judged on their teamwork and the success in achieving“ triple aim”: improving the individual experience of care; improving quality and the health of CMG’s population, and reducing the per capita cost of care for the population.

The physician is considered an integral component in the following:

Clinician recruitment, development, retention, management, and quality outcomes

Creating and maintaining a cohesive and cooperative work environment that supports teamwork among providers, front office, back office and all ancillary personnel at their centers that promotes the Patient-centered Medical Home model.

Creating a customer focused, service oriented work environment

Generating workflow efficiencies.

Advancing the full utilization of the electronic health record and digital imaging environments

Implementing programs and being involved with utilization management outcomes within the Pods

Maintaining well-functioning relationships with external specialty physicians and healthcare entities within their service area

The physician will work with their Operation Managers and Center Administrators to help maintain a single management entity that will be in charge of overall practice functions.

Ensures the quality of care and patient experience within the practices within their department.

Implements CMG utilization and financial initiatives and ensure their success.

Maintains functional relationships with external specialists and healthcare entities to optimize quality outcomes, patient experience, utilization metrics, and financial results.

Assists with the efforts to improve and maintain cost and utilization trends in his/her center(s).

Supports utilization of the electronic health records and digital imaging programs within their practices.

Maintains responsibility for physician productivity and appointment availability.

MD or DO degree. Board certification in a Primary Care specialty.

Five or more years of history in clinical practice is preferred.

Ability to drive results, work collaboratively, think strategically, manage external relationships
