This is a Temp position
To provide psychosocial assessments, therapies, crisis intervention and discharge planning to patients and their families.
To assist patients and their families develop a treatment and discharge plan for transition back to the community.
To coordinate admissions and discharges for the Geropsychiatric Unit.
DUTIES AND RESPONSIBILITIES
Coordinates intake for admissions,
Provides psychosocial assessments, written treatment and discharge plans in coordination with the overall treatment objectives for patients, including attention to age specific needs.
Obtains pertinent referral information from family members of the patient and gathers medical and psychiatric information from agencies and facilities.
Participates in treatment team meetings.
Plans and conducts group therapy sessions in collaboration with multidisciplinary team.
Refers to and collaborates with community agencies and clinicians to facilitate discharge.
Participates in Quality Management/Continuous Quality Improvement activities.
Master’s degree in social work required
Three years post graduate Social Work experience, preferably in acute
