Global Health Care Group is looking for a Licensed Physical Therapist (PT) to provide services at a Skilled Nurse Facility/Long Term Care Facility, located in Peterborough, NH. We are looking for an individual who is currently licensed in the state as a Registered Physical Therapist ( PT). Available Position: Licensed Physical Therapist (PT). Position Overview: The PT is responsible for the assessment of referred patients that require rehabilitation services which includes; providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation. Essential Duties and Responsibilities: ?Provides skilled physical therapy services/interventions in accordance with physician orders. ?Assesses patient needs, plans for, evaluates, and modifies care to meet goals of physical therapy interventions. ?Collaborates with all disciplines to plan and evaluate team goals for each patient.
Work Schedule: Fulltime, 40 hours per week. Period of Performance: Temp, Tem to Perm or Permanent Position. Position Requirements/Qualifications License: Must have a current, full, active, and unrestricted PT license in the state of NH. Education: Must have graduated from an accredited Physical Therapy school.
Certifications: Must have current Cardiopulmonary resuscitation (CPR) certification. Professional References: Must provide reference letters or reference contact information.
