Responsibilities - CUMC
This is an 1199 Union position.
Identifies and assesses patients’ needs to determine appropriate referrals and/or services needed. Analyzes reports to track usage and possible trends. Establishes and directs patient education efforts.
Bachelor’s Degree from an accredited college or university required. Clinical background, BA degree, Medical Graduate preferred. Two or three years experience in primary or community health required. Excellent oral and written communication skills required. Knowledge of community patient and family needs and of the Hospital and community resources to fill those needs required. Bilingual (Spanish – English) and knowledge of overall hospital, community health systems & resources required.
The hours for this position are as follows:
Sunday - 12am - 8am Monday - 12am - 10am Tuesday - 12am - 10am Wednesday - 12am - 12pm
