DESCRIPTION

Summary:
Provide Activities of Daily Living (ADL) and provide recreational activities to ADC clients.

Reports To: Director of Adult Day Health Care

Titles Supervised: n/a

Hours Per Week: Per Diem

DUTIES

Essential Job Functions:
1.Assist clients with ADLs which includes but is not limited to bathing, toileting, incontinent care, and ambulation. Serve meals and refreshments and assist or feed clients as necessary.

2.Participate in client recreational assessments, care plan meetings and perform routine client charting which includes, but is not limited to record of client weights, vital signs, pertinent progress notes and flow charts.

3.Lead client therapeutic recreational activities, individually or in groups as directed by Recreation Coordinator. Assist with preparing room for special events. Setup and prepare materials needed for own groups and teammates as needed. Encourage participation of all clients through motivational approaches.

4.Alert Activity Coordinator and/or Program Nurse to observable changes in client's mental or physical status.

5.Assist clients in and out of the building; including helping them on and off the bus.

6.Maintain kitchen area and records pertinent to meal preparation. Maintain inventory for kitchen and bathroom supplies.

7.Help maintain the ADC suite in a clean and orderly fashion.

8.Maintain regular attendance; perform other related duties as assigned.

QUALIFICATIONS

Minimum Qualifications:
Education: High School Diploma or GED; Certified Nursing Assistant or Home Health Aide

Experience: One year CNA experience in nursing home or acute care environment, or two years HHA experience. Experience in group activities, or special talent in music or crafts helpful

Licensure: n/a

Knowledge/Skills/Abilities: Ability to handle multiple demands and deadlines; must read and communicate effectively.
