As the Ryan White Program Outreach Coordinator, you will:
Be responsible for collaborating with the Ryan White program team to develop a comprehensive outreach plan.
Work collaboratively with the Director of HIV Services and other Ryan White progam staff to develop a comprehensive plan of outreach for the communities of Waterbury, Danbury, Norwalk, and Stamford.
Represent the Ryan White program on community boards and consortia in order to solidify community collaborations and will use community events to network and find partnerships.
Provide greater access to Ryan White program services in the communities that are currently underserved. Outreach will consist of street level outreach, community outreach and community events.
Participate in the design of printed materials, posters and handouts to use with partner agencies and potential clients.
Collect data related to outreach services; record, maintain and report outreach successes and developmental needs.
Work with HIV clinical staff at target sites to assist in community outreach and retention of clients.
Communicate regularly with clinical staff and Ryan White program staff about new patients.
Participate in Project ECHO on a monthly basis to enhance outreach and engage providers.
Attend monthly Ryan White team meetings.
Perform other duties as assigned.
Degree in a related discipline
Current certification in HIV/Hep C rapid C&T
Proven experience supporting clinical services through outreach
Strong communication, organizational, time-management, prioritization, multi-tasking, and interpersonal skills
Knowledge of communities to be served
Demonstrated proficiency in Microsoft Office and internet related applications
Bi-lingual skills preferred
Valid state of Connecticut driver’s license with good driving record
