We are seeking a committed and compassionate Patient Care Coordinator to manage our office in Henderson , NC to ensure that hospice care is provided to the ultimate satisfaction of patients and their families. As a Patient Care Coordinator with Community Home Care & Hospice (CHCH), you will meet the expectations of referral sources and other external customers and maintain the financial integrity of patient care. You will be responsible for effective communication, clinical appropriateness of care, adequacy and quality of patient care services, employee satisfaction, customer satisfaction and financial management. You will ensure that our mission, philosophy and values are upheld in the delivery of patient care services and participate with at least one of the following committees: Ethics, Safety/Infection Control, Quality/Utilization or IDG/PAG. You will also randomly audits invoices, IPOCs and medical charts to assure adherence to patient care plans and oversee the management and distribution of medical supplies.Additional tasks of the Patient Care Coordinator include: Assisting the interdisciplinary team with problem-solving regarding palliative care measures for pain and symptom management Reviewing all recertification recommendations before continuation or discharge and participating in all revocation discussions before discharge Developing and implementing procedure changes impacted by policy revisions that affect patient care team functions and making recommendations on policy changes Participating in on-call nursing care rotations Interviewing, hiring, training, supervising, evaluating and dismissing Team Members and Volunteer/Bereavement Managers Hiring maintaining a master list, evaluating and scheduling all per diems including alternate hours staff Providing orientation level training to new staff on palliative care practices Patient Care Coordinator – RN Registered Nurse Medical Manager Job ResponsibilitiesAs a Patient Care Coordinator with CHCH, you will act as a resource and mentor for Team Members and other department managers in clinical issues, regulations and other patient care and management topics. You will review and develop individual team human resource utilization, including volunteers and plans, to provide coverage mechanisms across teams that account for patient needs, staffing and census fluctuations. You will ensure that there is effective two-way communication between team(s) and other CHCH areas, help develop annual goals and objectives and serve as a mentor to newly hired Patient Care Coordinators. Quality improvement is always one of our highest priorities, and you will randomly monitor both active and closed charts to ensure that services are rendered according to patient care plans, that the team makes appropriate decisions that are responsive to patient and family needs and that documentation effectively demonstrates quality and regulatory compliance.Additional responsibilities of the Patient Care Coordinator include: Collaborating with Team Members to develop Performance Improvement plans and reviewing relevant data for quarterly Quality Assurance studies Reviewing all revocations, customer and employee satisfaction survey data and Customer Service/Complaint forms to analyze for areas needing improvement Preparing for survey process and annual program review by ensuring that company policies and federal, state and local regulations are followed Actively soliciting feedback on services from referral sources and making monthly contact with key referral sources Speaking with physicians, patients and their families to resolve grievances that the Team Manager has not been able to resolve and authorizing additional resources as needed Attending community functions, making presentations to community groups and professional organizations and conducting in-services for referral sources and contract facilities in conjunction with our Sales department Identifying opportunities for cost savings in patient care staffing and the utilization of ancillary services, assisting in the implementation of cost saving measures and assisting the General Manager in the budget planning process Patient Care Coordinator – RN Registered Nurse Medical Manager
