The Central Arkansas Veterans Healthcare System (CAVHS)Home Based Primary Care (HBPC)Advanced PracticeNurse (APN) serves as a case manager and primary care provider for a population of homebound, chronically-ill Veterans in Jefferson and Grant counties and the surrounding areas. HBPC is an interdisciplinary model of care that provides a blend of primary and palliative care to a largely geriatric population of Veterans. The HBPC APN provides comprehensive assessment, treatment planning, ongoing monitoring and care, patient/family education, and discharge planning. The APN works collaboratively with the other members of the team including the physician, rehabilitation therapist, social worker, clinical pharmacist, dietitian and chaplain. HBPC is an academic setting and the APN provides training for graduate nurses, graduate social workers, pharmacy students and residents, and geriatric fellows.
Relocation expenses will not be reimbursed.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: Accreditation Commission for Education in Nursing (ACEN) formerly known as The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Must be proficient in spoken and written English.
Current prescriptive authority in the State of Arkansas
Certification as an Adult Gerontology, Adult or Acute Care Nurse Practitioner.
Current certification in BLS
Valid state driver's license
Applicant's education and experience will be reviewed by a Professional Standards Board in determining the grade and salary of the applicant selected. The recommended salary may be at any point in the range listed for this vacancy.

Experience:
Five(5) years ofAdvanced Practicenursing experienceis preferred.
Three(3) years ofAdult/Geriatricprofessional (RN) nursing experience is preferred.
Home Health professional (RN) nursing experience is preferred.
Case Management experience strongly preferred.
