Berlin Nursing and Rehabilitation is seeking a Part Time Activities Aide. Please read description below:
The Activities Aide provides therapeutic activities to residents that are designed to meet the interests and to improve the physical, mental and psycho-social well being of each resident according to the comprehensive resident assessment and plan of care. Individual resident programs assist the resident to maintain lifelong interests and self-esteem while promoting physical, cognitive and emotional functioning. The Activiteis Aide provides activities in accord with the facility policies and procedures and consistent with state and federal laws and regulations.
Carries out individual and group activities that meet the functional levels, needs and interests (including cultural and religious interests) of each resident. Follows the planned calendar of recreation opportunities.
Provides activities to residents requiring individual interventions that reflect life-long interests (such as music, reading, and visits with people of shared interests) and that are adapted to the individual''s functional abilities and level of participation. Assists residents with these activities as needed.
Gathers data from assigned residents to complete and update activities assessments and plans of care. Actively prepares for and participates in the plan of care meetings.
Completes documentation for assigned elders with quality insight, accurate information and in a timely manner.
Seeks ands identifies ways to support resident''s individual needs and preferences, customary routines, concerns and choices. Communicates this information to immediate supervisor and other caregivers as appropriate.
Adapts activities to match the cognitive and physical functional levels of the residents through the use of task segmentation, verbal prompts, set-up assistance, physical assists and demonstrations.
Assists Director of Therapeutic Recreation with development of activity calendars. Posts calendars on resident units and distributes these schedules to residents.
Monitors physical environment for safety or fire hazards and corrects or reports potential problems ultimately to unit charge nurse. Promptly reports housekeeping and maintenance problems to unit Charge Nurse.
Replaces all activities equipment and supplies in the proper storage areas. Maintains activities space in clean and orderly manner that appears "homelike" and is safe for the residents, including proper placement of furniture and orderly storage of equipment and supplies in drawers, cupboards, closets etc. Performs additional cleaning tasks as assigned by Director of Therapeutic Recreation.
Assists Director of Activity department with preparation for state inspections. Interacts with long term care state or federal surveyors as instructed by immediate supervisor.
Attends survey training and all required in-service education offerings. Applies information learned to daily job tasks.
Reports injury to resident or self to department supervisor or house supervisor immediately.
Records information in resident records according to facility policy and as instructed by immediate supervisor.
Participates in fire and disaster drills. In the event of an emergency carries out assigned duties to assure resident safety.
Must work the days and hours necessary to perform all assigned responsibilities and tasks. Must be available (especially during regular business hours or shifts) to communicate with supervisors, customers, vendors and any other person or organization with whom interaction is required to accomplish work and employer goals.
Must be punctual and timely in meeting all requirements of performance, including, but not limited to, attendance standards and work deadlines; beginning and ending assignments on time; and scheduled work breaks, where applicable.
Must maintain courteous, professional and effective working relationships with employees at all levels of the organization.
Must develop and maintain courteous and effective working relationships with clients, vendors and/or any other representatives of external organizations.
Must assume responsibility for providing timely, appropriate responses to requests/suggestions/complaints or refer such comments to the appropriate person. When referring requests/suggestions/complaints to another individual, the employee must follow-up in writing to the individual and must maintain an accessible copy of the referral, or follow company guidelines.
Must communicate information and state problems or challenges to be resolved in a clear, concise, courteous, nondiscriminatory and professional manner, and be able to provide clarification, as necessary.
Must show respect for the opinions of others and behave in such a way as to ensure an atmosphere free of needless interruptions, difficulty and/or discrimination.
Must receive and provide clear, constructive feedback regarding work performance (to/from individuals or groups) and be able to clarification, as necessary.
Must handle in a constructive, non-disruptive manner, the multiple business, organizational and interpersonal changes or stresses that may exist in the employee''s work, work processes, dealings with other personnel, and supervision (provided or received). ("Work processes" include but are not limited to, how work is performed, supervised or distributed: how problems are solved; and how decisions are made.)
Must be sufficiently adaptable to accept and perform in a timely and effective manner work assignments that are outside the normal (day-to-day) routine.
