The CCH Case Management Manager is responsible and accountable for providing Uncompromised Safety, Superior quality, Memorable Patient/Customer Experiences, and Financial Stewardship by providing the leadership and management component to the overall Case Management Process. The primary functions include Leadership, Facilitation, Performance Improvement, Data Management, Human Resource Management, Fiscal Management, and overall facilitation of Case Management Services. These functions are carried out while maintaining quality and achieving desired outcomes/goals. Primary Accountabilities •Maintaining 24-hour accountability for Case Management Services at CCH to ensure department goals are achieved while maintaining patient quality in a cost effective and efficient manner. •Directs the Case Management teams to ensure clinical, customer/patient satisfaction, and financial goals are achieved. •Provides ongoing coaching and staff development to ensure performance of Case Management Team Members. Provides leadership through role modeling. •Serves as expert in fields of case management and managed care, interpreting and communicating processes and changes in these fields to staff, physicians, and the community. •Facilitates Case Management Team involvement in the establishment, implementation, coordination, modification, and optimization of a timely, collaborative, appropriate and patient focused plan of care. •Ensures overall CM staff compliance with regulatory and accreditation agency standards of care, both State and Federal and JCAHO. •Serves as a catalyst for organizational change to improve operational and clinical processes that impede patient progression to optimal outcomes. •Plans, develops, implements, and monitors all financial management activities including, but not limited to budgeting and staffing, under the direction of the VP of Medical Affairs.
      Qualifications
      •Current RN License
      •Bachelors Degree required; Masters Degree preferred.
      •Minimum 5 years experience in acute care setting
      •Previous management/leadership required
      •Able to conduct meetings and speak in front of large groups
