Ambulatory Care Pharmacist Specialist
actively participates in a pharmacist-managed, point-of-care
anticoagulation clinic and provides support for the internal medicine, diabetes, and transition of care clinic
. This Outpatient Clinic serves is a teaching clinic and serves as a training site for medical residents.
Promotion of safe, appropriate, and cost effective medication therapy is a key role, including the use of patient assistance programs when applicable.
This Specialist will support the training of students and PGY1 and PGY2 residents as a core component of their respective curriculums.
The Ambulatory Care Specialist will also participate in hospital committees and other duties as assigned.
Requires Pharmacy Degree from an accredited College of Pharmacy or foreign equivalency. Requires Completion of a PGY-1 Pharmacy Practice Residency.
Completion of a PGY2 in Ambulatory Care or equivalent experience is preferred.
Requires current State of Michigan Pharmacist and Controlled Substance licensure.
