Basic Function: Provides community and home based care to at risk families and children. Assists family in identifying a Primary Care Physician and assists with linkage to their medical home. Refers to, and works closely with, appropriate community resources to assure linkage of the family and child to appropriate coordination resources and delivery of appropriate services.
Requirements for this position include:
Bachelor's degree required in a social services field, or meets the “Mental Health Professional” designation as determined by the Department of Human Services.
Resident of, or individual with familiarity of, project neighborhood in Springfield, IL, (Cook St. to S. Grand and 11 th St. to 18 th St.), highly desirable.
Related experience preferred.
Must have keyboarding and computer skills as required by position.
Must have valid IL Driver’s License, reliable transportation, and proof of auto insurance.
Must have or obtain CPR, First Aid, and Personal Safety certifications.
Must be able to pass fingerprinting and drug screening.
