Assumes the responsibility for coordinating and
facilitating services for patients during hospitalization. The
RN Case Manager conducts admission, concurrent and retrospective
review, and discharge planning to effectively move the patient
through the continuum of care to promote high quality healthcare in a
cost-effective and resource efficient manner.
RN license in the District of Columbia (or licensed
eligible); minimum of 3-5 years of clinical experience in an acute
care setting; knowledge and experience with computers is required;
noBSN and/or preious experience in case management, discharge
planning or utilization management is strongly preferred; Utilization
Review/Management and/or Case Management Certification
