Job Summary :
The ED Care Coordinator will work directly with the clinicians in the Emergency Department (ED) to ensure the delivery of quality, cost-effective health care in the most appropriate setting. He/she will work directly with the ED physician to assure that an order to admit is secured, that the order matches the admission status assigned to the patient by the physician, a nd that both are consistent with Interqual criteria. Entries into the BCBS portal and Midas will be done on all admissions, regardless of status. The ED Care Coordinator will also provide assistance to the care team in post-discharge planning and implementation of the plan.

Education/Experience : RN graduate of accredited school of nursing; BSN or BS in other h ealth related field preferred. Two to five years of experience in care coordination required. Previous ICU experience preferred.

The care coordinator must demonstrate personal traits/behavior consistent with the core values of St. Vincent's Hospital. Extensive knowledge of interqual criteria must be demonstrated by successful completion of the McKesson inter-relator reliability test exhibiting a score of 85 or >.
