Position Summary
In this position, the Clinical Performance Improvement Coordinator is responsible for activities including, but not limited to, data abstraction, core measure data analysis, and hospital wide education.

Essential Duties and Responsibilities
Essential Duties
¨ Performs data abstraction requirements according to CMS and JC specifications for Core Measures.
¨ Analyses Core Measure data, identifies opportunities for improvement and communicates results to key stakeholders.
¨ Facilitate Core Measure Performance Improvement Teams to identify and resolve opportunities for improvement.
¨ Prompts physician and care givers to perform tasks or document exceptions to improve compliance.
¨ Provides hospital wide core measure education; including new employee orientation and continuous education to care givers and physician.
¨ Communicates bi-annual updates and coordinates new process development as necessary.
¨ Acts as a liaison between Premier, Quality Net Exchange, and Mountain Pacific Quality Health Foundation to ensure data completion and accuracy according to specified deadlines.
¨ Research and communicate evidence based practice.
¨ Resource for staff and physicians regarding the elements that comprise publicly reported data and external documentation initiatives.
¨ Collaborates with Data Specialists to create automated data abstraction processes and maximize tools such as MIDAS.

Education, Experience and Licensure/Certification Requirements: Requirements are
representative of the minimum level of knowledge, skills and/or abilities necessary to perform the essential functions of the position Current MT RN license Minimum of three years experience in the healthcare field Ability to communicate effectively and diplomatically within a multi-functional team Ability to successfully function in a fast paced, service oriented environment Strong organizational skills and attention to detail Experience in understanding and usage of computers, including the Microsoft Office Suite, as well as the abiltiy to learn applications relevant to the position
