Under limited supervision, manages the day to day program planning, budgeting, staffing, supervision, training and in-services and program evaluation of Health Programs in assigned department.

Coordinate substance abuse, clerical, case management, outreach and social services to clients of all ages; serves as a member of a collaborative Homeless Health Care Center Management Team; provides personnel administration of all case managers, social workers, and support staff; develops programmatic and administrative policies and procedures; acts as a liaison with the Community Kitchen and Johnson Mental Health Center; participates in work groups whose goal is to plan and organize the scope, objectives, and methods of service delivery at the health center; assists assigned staff, co-workers, and cooperating agencies in the delivery of client services; monitors expenditures and approves requests for purchase; orders related supplies and equipment; seeks community resources; establishes and maintains referral mechanisms with other community agencies; represents the project through the media and speaking engagements; represents the project at community coalitions, action groups, etc; networks with community agencies; coordinates the structure and meetings of the Homeless Health Care Center Advisory Board in collaboration with the Homeless Health Care Clinic management team; implements the project plan including primary care, substance abuse services, emergency health services, outreach services, eligibility assistance, and referral for mental health services; directs the work team to achieve the goals and objectives of the Health Care and Business plan; provides social service and counseling activities including assisting clients with housing, food, shelter, entitlement programs, employment, counseling, developing patient care plans, securing identification and intakes; coordinates and monitors scheduling, assignments, and attendance for all assigned staff; assumes responsibility for operations of entire health center in the absence of the Clinical Coordinator; promotes a safe and effective work environment by following OSHA regulations, standards of care, and agency protocols; monitors service delivery and productivity of assigned programs and makes adjustments as necessary; works with the Homeless Health Care Center management team to maintain Joint Commission accreditation for both ambulatory care and behavioral health care; assures compliance with external licensure requirements for assigned programs; evaluates client flow for efficiency and makes adjustments/improvements as necessary; works cooperatively with the Medical Director and Clinical Coordinator in hiring staff, program planning, scheduling of activities and development of policies and procedures; shares appropriate information with the Medical Director, Clinical Coordinator and Primary Care Program Manager; coordinates and plans weekly staff meeting in collaboration with the Homeless Health Care Center management team; responsible for facility management and compliance with JCAHO Environment of Care guidelines; develops, implements and revises facility management plans; leads the Environment of Care Committee and serves as the Home Health Care Center’s Safety officer; coordinates and supervises all case management students; serves on the Performance Improvement Committee and the Case Management Record Review Committee; demonstrates effective communication and interpersonal skills with all staff.

Knowledge of federal, state and county laws, rules, regulations, policies, practices and procedures related to Public Health and Community Health program and service provision.
Knowledge of Health Program issues in geographical area.
Knowledge of federal, state and county laws, rules, regulations, guidelines related to safety and health practices and procedures.

Knowledge of customer service/public relations methods, practices and procedures.

Skill implementing and providing oversight to health programs and services to the indigent, underserved and homeless populations within Hamilton County.

Skill utilizing customer service/public relations techniques responding to inquiries and complaints. Skill assessing client needs and establishing treatment plans.

The equivalent of a Bachelor’s Degree in Public Health, Health Care Administration, Social Services, or a closely related health field with three (3) years of health care program experience in area of assignment, one (1) year of which must be in an administrative, supervisory capacity.
Physical Requirements: Work requires extended periods of concentration on detail, some work outside standard business hours when doing presentations and public speaking engagements, possible exposure to infectious diseases, blood, air and water borne pathogens when performing site or home visits.
