New Directions for Veterans has an immediate opening for a Housing Stability Case Manager located at our newly constructed Sepulveda location (Panorama City, CA). The Housing Stability Case Manager will work onsite at New Directions Sepulveda, our permanent supportive housing project, to provide comprehensive, tenant-centered supportive services to formerly homeless Veterans. The Case Manager is responsible for providing individualized service planning, advocacy, hands-on support, education, care plan coordination, and collaboration with other providers to an assigned group of tenants. The Case Manager provides specialty group and individual counseling as assigned and is responsible for maintaining computerized case management and hard copy files.

The selected candidate will possess the ability to:
Collaborate with tenant, other staff, providers and care-givers in the creation of individualized housing retention plans for each tenant that incorporates best therapeutic practices.
Provide individual support, coordination and linkages to community resources in the areas of job training, education, money management, income, benefits attainment, medical, legal, dental, substance use and mental health services.
Work alongside tenants in the day-to-day activities of independent living, teaching and assisting with cooking, bill payment, and house-keeping.
Utilize therapeutic practices such as Motivational Interviewing, Strengths-based Case Management, and Trauma Informed Care to assist tenants with self-sufficiency and stabilization.
Travel with tenant to appointments when necessary.
Work consultatively with other staff to ensure appropriate levels of treatment/support
Keep tenant files current through the computerized care management system and maintain ongoing files both on computer and hard copies.
Participate in outreach to identify homeless veterans and present the program as an alternative to homelessness.
Liaison with the VA outreach departments, community outreach departments, VASH department and the VA benefits and hospital departments
Participate in conferences, workshops, special projects, staff meetings, and other duties as assigned.
Provide and participate in activities that foster community and celebration.
Cultivate relationships with other non-profit and for profit developers, cities and counties and explore new partnerships for housing development and services funding opportunities.
Work with NDI program staff to identify and apply for funding for service delivery in permanent supportive housing.
Work with Executive Director, VA and local housing authorities to secure sponsor-based and project-based VASH allocations.
Work with CAVSA and other policy coalitions on legislative changes
Participate in agency strategic planning by reviewing all existing projects and areas of the county most underserved and develop a plan to minimize cost or management and

Job Requirements:
Knowledgeable of mental health and substance use issues, therapeutic interventions, and treatment planning, particularly as relevant to Therapeutic Communities
Working knowledge of community resources as they relate to homelessness and veterans
Experience and knowledge in working with a multidisciplinary treatment team
Word processing skills required; HMIS skills preferred
Veterans preferred
Housing experience preferred.
Three (3) years of direct case management work experience is preferred
Bachelor's Degree is preferred.
