The Home Health Aide/Hospice Aide is a trained person who provides personal care and assistance in activities of daily living for people in their place of residence. These services are included in a written plan of care prepared by a health care professional and signed by a physician. The Home Health Aide/Hospice Aide works under the direct supervision of a Registered Nurse and/or when appropriate a Physical, Occupational or Speech Therapist under the delegation of their License. Must be willing to travel to the Chippewa Falls, Menomonie, and Osseo service areas to care for patients.
Ability to read and communicate effectively in English. Basic computer skills and experience with other technological devices. (i.e. cell phone, black berries, etc…). Ability to work irregular hours (days, evenings, holidays, weekends). Self motivated. Able and willing to drive in all weather conditions. Able and willing to work with all populations served.
High School Graduate or GED equivalency preferred. Meets Federal and State guidelines required to work as a Home Health Aide in a certified licensed Home Health agency. Has worked as a CNA within the past 24 months preferred. 18 years or older preferred.
License or Certification:
CNA on State of WI Registry. Current BLS certification or the ability to be certified prior to hire date. Valid WI driver’s license.

Exemption Status: Non-exempt
Hours/Pay Period: Full time ; 60 - 80 hours per pay period
Schedule Details: Monday - Friday ; day & evening shifts. Holiday Rotation.
Weekend Schedule: 1 of 2 Weekends
Compensation Detail: Education, experience, and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $11.28 per hour.
