POSITION SUMMARY
The physical therapist is responsible for providing physical therapy Services according to accepted standards of care, physicians plan of care, and home health agency policies. Additionally, when physical therapy Services are provided under contract, the physical therapist adheres to all conditions in the Contract/Agreement. Performs the initial physical therapy evaluation; and supervises the PTA no less than weekly in acute situations, and no less than monthly, otherwise.

REPORTS TO: Director of Professional Services

QUALIFICATIONS
1. Possesses a degree from a baccalaureate or masters program in physical therapy approved by The American Physical Therapy Association; or The Committee on Allied Health Education and Accreditation of the American Medical Association; or The Council on Medical Education of the American Medical Association and the American Physical Therapy Association.
2. Is licensed to practice as physical therapist in Texas.
3. Is knowledgeable of accepted professional standards and practice.
4. One year of appropriate experience as a physical therapist and community/home health experience is preferred.
5. Has good oral and written communication skills; has good organizational skills.
6. Possesses and maintains current CPR certification.
7. Supervises Physical Therapy Aids, Home Health Aides
8. Coordinates with Patient Care Staff

RESPONSIBILITIES AND DUTIES

  • Assesses and evaluates the patients therapeutic/rehabilitative/functional status, including muscle strength, mobility, gait, ROM, and potential for rehabilitation.
  • Evaluates home environment.
  • Participates in the development of the total plan of care.
  • Initiates physical therapy program and instructs other personnel and/or family/caregiver members in those parts of the program in which they may participate.
  • Instructs patients/families/caregivers in the use and care of therapeutic appliances and prosthetic devices.
  • Includes patients/families/caregivers in planning and achieving goals of the physical therapy program.
  • Reports to physician patient's reaction to treatment or changes in condition.
  • Supervises HHA's when such personnel are participating in patient's rehabilitation regime.
  • Identifies patient and family/caregiver needs for other home health Services and refers as necessary.
  • Prepares and submits clinical notes and progress summaries based on the attainment of goals, and according to agency policy.
  • Provides physical therapy consultation to home families/caregivers when indicated.
  • Provides in-service education programs for nursing, and other, agency personnel as needed.
  • Participates in peer consultation process.
  • Performs other duties as required to facilitate the delivery of physical therapy services.
