DESCRIPTION
The primary purpose of this position is to effectively manage cases and to coordinate referrals with outside medical facilities, hospitals, clinics, and agencies, in order to provide the appropriate patient care. Is involved in the day-to-day scheduling for specialty appointments and confirmation of appointments. Completes necessary authorization forms. This position also promotes community awareness.
DUTIES
Coordinates patient referrals to outside specialists, physicians, testing facilities, agencies, and support/advocacy services.
Schedules and confirms appointments for patients at specialty clinics, as necessary.
Refers patients to appropriate sources for home health care.
Coordinates transportation needs, which includes providing maps, distributing bus tickets, and maintaining appropriate records.
Initiates and maintains tracking data related to referrals and lab/studies; assures that reports are available for primary providers.
QUALIFICATIONS
Job duties require the equivalent of a high school education, with specialized courses equivalent to a two-year Associate Degree in accounting/bookkeeping, and with the appropriate training in business, spreadsheets, and Microsoft/Windows computer skills. Must maintain effective written and oral communication/English skills. Must maintain a current NC driver's license for outside travel.
Spanish skills a plus!
