The Medical Social Worker is a qualified professional who provides medical and social services to patients in the home with the physician''s orders and under the supervision of the Clinical Nurse Case manager. This position requires the ability to assess, treat, care for, understand and react effectively to the patient population for whom services are provided (infant, pediatric, adolescent, adult and/or geriatric).
Graduate of a school of social work approved by the Council of Social Work Education with a Master''s Degree.
Registration/Certification or License Required:
C.S.W. certification by the state preferred.
Heartsaver CPR (or completed within 60 days of hire)
One year experience in a health care setting (hospital, clinic, rehabilitation center, etc.).
Management Experience (If applicable):
Reliable means of transportation.
Reliable means of communication.
