NURSE CASE MANAGER $4,907-$6,025/month, 37.5 hour work week Closes: Friday, February 4, 2011 at 5:00 pm The City of Oakland's Department of Human Services is seeking 2 RNs to perform case management duties involving health assessments on frail, elderly and low-income program applicants. You will also be expected to monitor health status of clients, make referrals when appropriate, maintain complete and accurate records and perform related duties.

Minimum requirements include a current CA RN license, an Associate's degree from an accredited college or university in Nursing and 1 year of nursing experience. Bilingual skills in Spanish, Cantonese or Mandarin are desirable.
