Transitions Specialist (Sun & Tue Off)

Position Overview:
The primary task of the Transitions Specialist is to assist youth within an RTC setting develop and achieve a successful transition into young adulthood that may include living independently. Staff will carry out assigned duties and responsibilities in a therapeutic and supportive manner with older youth with emotional and psychiatric issues. Staff shall be responsible for living skills assessment; development and implementation of individual service plans; carry out life skills development workshops; promote self advocacy; build and support job readiness skills; promote savings and financial responsibility; promote education/vocational development; as well as create and support an environment that is outside of the typical RTC model. Work includes assisting with discharge planning and other duties as assigned.

Position Qualifications:
Associates degree with at least two years counseling or independent living experience OR Bachelor’s degree in human services field required. Valid driver’s license, required. Experience in independent living skills development. Experience working with at risk foster care youth. Experience in running groups/workshops, preferred. Bi-lingual (English/Spanish), ideal.

Salary $28,000
