Graduate of accredited seminary or school of theology and or training in healthcare chaplaincy. Certificate in Pastoral Education.
Experience in working with patient and families dealing with life threatening illnesses and death preferred.
Completion of a minimum of one (1) unit of clinical pastoral education or has the equivalent pastoral experience or as required by state specific guidelines.
Minimum two years of ministerial experience.
As a member of the interdisciplinary team, the chaplain is responsible for the provision of ecumenical and inclusive spiritual support to the patients, families and staff
Responsible to assess and re-assess the spiritual needs of families.
Provide support on an ongoing basis, in accordance with hospice standards of care. Assume responsibility for a patient/family caseload.
Complete assessment of spiritual needs/concerns of patients and families by direct communication with the individuals, by communication with the patient’s clergy, or by information obtained by other hospice team members
Assist in maintaining a program of care to assess spiritual/emotional needs and concerns of patients and families.
Conduct or assist with arrangements for funerals or memorials services when requested
Complete clinical documentation according to agency, state and federal standards
