ESSENTIAL JOB FUNCTIONS
PATIENT TRIAGE MANAGEMENT:
-Responsible for answering a multi line phone system efficiently.
-Coordinate with hospitals, internal and external providers to ensure continuity of patient care.
-Provide education to patients and/or caretakers regarding procedures, medications, lab test results, etc.
-Use independent discretion with patients to determine level or urgency in emergent situations; schedule appointments or refer patients to emergency room when needed.
-Facilitate and promote full cycle health maintenance, wellness programs, and heart disease management for patients.
PATIENT SAFETY AND SECURITY:
-Follow patient safety-related policies, procedures, and protocols
-Demonstrate proactive approach to patient safety and hazards; report them promptly
-Identify and report/correct environmental conditions and/or situations that may put a patient at unnecessary risk
-Report potential or actual patient safety concerns and medical errors in a timely manner
-Participation in special projects, in-services, and scheduled nurse meetings
-Cover in other departments when needed
-Work independently and in a team setting.
-Training and cross training of team members as needed.
-Other duties as assigned.
-High school diploma or GED required
-Graduate of NLNAC accredited school required
LICENSURES OR CERTIFICATIONS:
-New Mexico RN licensure required
-BLS and ACLS certification required
-Minimum two (2) years nursing experience in cardiology required; ER experience a plus
-Demonstrated experience working with high level professionals requiring outstanding expertise and professionalism.
-Experience working with diverse populations with limited understanding of insurance or healthcare processes and procedures.
-Problem solving skills such as researching information; the ability to determine if information found is valid and the ability to make decisions with information gathered.
-Bilingual in Spanish or local Native languages (oral and comprehension) a plus.
-Impeccable active listening skills to notice non-verbal clues over the phone
-Must possess strong organizational, time management, detail and communication skills; the ability to prioritize projects and work in a fast paced, frequently crisis driven environment.
-Proficient computer skills required, including Microsoft Office Suite, e-mail, and typing; expert in spreadsheet usage a must.
-Must be able to provide complicated information in a clear and concise manner to those not familiar with the information being provided.
