Requires an associate’s degree and/or certification as a Registered Health Information Technician (RHIT).

A nursing degree, or comparable medical degree will be considered with CCS, or CPS coding certification.

RHIT, CCS, CCA or CPS coding certification

years experience coding with ICD-9-CM and CPT-4 coding classification systems, and 1-2 years working experience in a Medical Record Department.

A medical records coder/abstractor abstracts clinical information from a variety of medical records and assigns appropriate ICD-9-CM and/or CPT-4 codes to patient records according to established procedures.

Assures that all services documented in the patient's chart are coded.

Responsible for DRG and API assignment for Medicare Patients.

Complexity and Difficulty:
Ability to work independently with minimal supervisory direction, and as a team player.

A certain degree of creativity and latitude is required.

85% Demonstrates a 95% accuracy rate of assigning the principal diagnosis and any procedures, coexisting complications or co-morbidities for each medical record.

Analyzes, enters and manipulates the database, confirms appropriate DRG or APC assignments to assure accuracy, maximum reimbursement and statistical gathering.

Adheres to coding guidelines and corporate compliance plan.

15% Using the MediTech generated “End of Month” report, identifies uncoded accounts that need to be completed; Using the MediTech generated “Uncoded"

report, identifies uncoded accounts that need to be completed; Works with members of the billing department to assure maximum efficiency and reimbursement for properly documented services; Reviews, modifies, and recommends changes to policies and procedures to improve coding and data management; Maintains continuing education requirements and reviews coding periodicals for changes in requirements, e.g. Medicare, BCBS, etc.; Networks with other coders to share knowledge and learn from each other.Works with members of the billing department to assure maximum efficiency and reimbursement for properly documented services; Reviews, modifies, and recommends changes to policies and procedures to improve coding and data management; Maintains continuing education requirements and reviews coding periodicals for changes in requirements, e.g. Medicare, BCBS, etc.; Networks with other coders to share knowledge and learn from each other.
