The position of Healthways Hawaii Well Being Improvement Center (WBIC) Director of Care Management (DCM) will report to the Hawaii Business Leader /VP, consistently directing and overseeing the objectives and goals of the PCMH based Well Being Connection Integrated Care Model.

The DCM directly leads and manages teams of colleagues including Case Management, Clinical and Wellness Coaching, Quality Operations, Government programs and Colleague Training. The DCM provides guidance, leadership and supervision to the team. The DCM assists to ensure with the partnership of the customer (HMSA) that the quality, clinical, and operational performance goals of the Healthways HI and HMSA Integrated Health Management services contract are being met. In addition to playing a key leadership role in meeting the shared HMSA "Drive to Excellence" goals. The DCM is responsible for operational processes and initiatives that support achievement of performance goals. The DCM is responsible for the colleague development and overall performance management of the team based upon feedback from the customer and communicates effectively with his or her direct reports, peers, and the HI business leader.

Key Responsibilities:
Manage; lead an integrated care delivery team of 100+ colleagues who will work collaboratively with community based physicians (PCMH model).

Provides consistent development and retention of WBIC team with the support of Human Resources, Trainers, and other support staff.

Provide performance management for the team of assigned functional team leads, clinicians, Health coaches, PSA's and trainers to include formal performance evaluations and delivery of performance feedback in collaboration with Medical Director and the field based PCMH delivery team.

Continually evaluate performance against objectives for the team, develop strategies and implement initiatives to achieve performance goals.

Use continuous improvement methodologies to develop and implement innovative approaches to provide, expand or improve the current HMSA contract, including finding creative solutions to Hawaii based issues. Employ new best practices across all functional groups as appropriate.

Collaborates with the Team leads, Medical Director, Pharmacy Leader, Training Manager, Well Being teams and HR, as well as other internal colleagues, to monitor overall delivery performance to ensure that contractual requirements are being met or exceeded.

Participate in new hire recruiting process of team members.

Participate in quality improvement activities as required by customer and HH Business Leader.

Interact with HMSA and provider community in a variety of areas such as Well Being and integration activities.

Provide clinical/coaching oversight to team to ensure consistency in service delivery to drive favorable results for shared HMSA DTE goals.

Appropriately utilizes diagnostic tools and reports to identify feedback opportunities for colleague performance improvement and recognition.

Participate in operational activities as directed by Healthways HI business leader.

Interact and develop partnership with the HMSA Operations and Quality team leads at appropriate levels.

Participate and lead both internal and customer meetings and trainings.

Qualifications

This position requires a self directed, self-motivated, committed individual with managed care, strong leadership skills, and excellent communication skills. The successful candidate will recognize, embrace and support the philosophy, mission, values and vision of Healthways. This key individual will be able to demonstrate these values with his/her leadership practices. This individual will also possess both the integrity and demeanor consistent with professionally representing. Strong analytical and decision-making skills are important.

Individual will have a minimum of five to seven years of progressive leadership experience, preferably in Care Center Service operations or as a Managed Care Organization business operations leader.

The successful candidate must possess a Bachelor's degree in business management or related field with a Master's degree a plus.

Prior ACO/PCMH care delivery experience preferred.

An appropriate business background with 5+ years of management experience including supervising a team of remote, field clinicians is preferred.

Prior management and supervision of a clinical team is required.
