This job has a dual focus, with responsibility for providing professional nursing care to patients with alterations in skin/tissue integrity (e.g. pressure, vascular, arterial and diabetic ulcerations, as well as skin conditions resulting from incontinence, surgical procedures and/or trauma) and for managing/coordinating patient case/wound management needs including identification, assessment, prevention and continuing education to achieve the best healing outcomes in an outpatient environment. An incumbent is assigned to specific physicians/clinics, and performs both patient care and case management activities according to established clinical pathways and within scope of practice. Work includes intake/discharge planning activities, initiating/monitoring the quality management program, obtaining insurance authorization for clinical procedures and coordinating interdisciplinary teams that include healthcare practitioners, rehabilitation therapists, social workers and other healthcare disciplines or providers depending upon individual patient assessments. Work is performed under standards of safety and care that provide instruction/guidance for taking care of problems and patient needs in the absence of a physician. Requires the use of judgment and critical thinking skills in making decisions regarding patient care (within the scope of practice), including knowledge and integration of available standards, resources and data, and in the efficient utilization of staff/resources. Work involves continuous contact with physicians, patients, patient families, community agencies, patient care staff and management.
Delivers and coordinates care to patients within the scope of practice and under the direction of physician or other clinician; performs patient intake, initiating clinical pathway on admission; collects patient history and develops nursing assessment; reviews charts, lab reports and other diagnostic/treatment data; contacts clinician to get orders; reviews assesses, measures and evaluates wounds and wound progress/status; interprets diagnostic data; assigns grade/staging of wounds; photographs wounds; cleans/irrigates wounds, determines appropriate dressing, and applies or changes dressing as required; interprets data to determine when a change in care is appropriate and makes decisions regarding treatment changes within defined scope of responsibility.
Participates in rounds/examinations conducted by physician/clinician during clinic hours to develop/confirm patient plan of care; prepares patient for visit; observes/documents wound care treatment and identifies next steps for the patient in accordance with the established clinical pathway and overall plan of care; may assist with debridement and other wound treatments as requested.
Coordinates patient care in accordance with established standards, strictly defined timelines and overall plan of care; identifies, communicates, tracks and monitors all steps along the continuum of care and maintains plan of care; ensures that lab/diagnostic work is done and that appointments/referrals (e.g. Hyperbaric, vascular studies etc) are scheduled as necessary; follows up to ensure continuity of medical care; maintains contact with patient's clinician and advises other care providers of developments in patient's condition when indicated.
Answers questions and provides health care education and guidance, in collaboration with interdisciplinary care team, to patients and/or caregivers in wound care management; documents patient education; explains/interprets results, findings and diagnoses to patient and/or patient's family within standards of practice.
Graduate of an accredited school of nursing.
Bachelor of Science in Nursing is preferred
Two years of related work experience working with an interdisciplinary clinical team in an acute care setting in one or more of the following clinical areas:
Current State Licensure as a Registered Nurse required.
Current professional certification (e.g. WCC, CWON, CWS or equivalent) is preferred.
If classified as
RN-Wound Care Coord, Certified
Current certification as a Certified Wound Care Nurse (CWCN) is required.
