As the Health Planning Consultant you will perform case management duties coordinating the medical care of inmates living with communicable diseases and major medical conditions who are being released from prison; indentify treatment plans and prepare discharge planning; establish time lines for provisions of services; develop relationships with community agencies and serves as liaison; interface with County, State, Federal and Tribal Health Departments to ensure continuity of care; provide education regarding health services needs to ADC staff and inmates; assist in providing and/or developing health fairs; attend training and workshops; and performs other duties appropriate to the assignment.
KNOWLEDGE, SKILLS, AND ABILITIES (KSAs): KNOWLEDGE: Of policies, procedures and practices of Health Services; local community, state, federal health and mental health agencies; nature and causes of medical and mental illnesses; health program administration; communicable and chronic diseases; communicating with culturally diverse population and education training protocols.
SKILL: In case management; establishing and maintaining controls over case plans; interviewing and assessment; organizing; tracking; time management; program planning and design development and speak and write effectively.
ABILITY: To track, identify and document inmates requiring transitional care, plan and organize health programs; analyze data and draw logical conclusions, identify the medical care and community services required by the inmates and ensure inmates have access to the services upon release.
SPECIAL SELECTION FACTOR: Will possess a Bachelor's Degree in Health Education, Public Health, social science, psychology, counseling, or in a related social service field and four years of experience in health planning or in a closely related field. Master's degree may be substituted for two years of required experience.
PRE-EMPLOYMENT REQUIREMENT (S): Employment is contingent on the successful applicant passing a background investigation, pre-employment drug test, and a medical/physical examination.
RETIREMENT: Positions in this classification participate in the Correctional Officer Retirement System (CORP).
