Job Summary: Provides professional nursing care to patients with alterations in skin/tissue integrity (e.g. pressure, vascular, arterial and diabetic ulcerations, as well as skin conditions resulting from incontinence, surgical procedures and/or trauma). Manages/coordinates patient case/wound management needs including identification, assessment, prevention, and continuing education to achieve the best healing outcomes in an outpatient environment. Work includes intake/discharge planning activities, initiating/monitoring the quality management program, timely and accurate completion of patient and billing documentation, and coordinating interdisciplinary teams that include healthcare practitioners, rehabilitation therapists, social workers, and other healthcare disciplines or providers depending upon individual patient assessments.
Essential Duties: Delivers and coordinates care to patients with alterations in skin/tissue integrity within the scope of practice and under the direction of physician or other clinician. Performs patient intake, initiating clinical pathway on admission. Collects patient history, reviews chart, lab reports, and other diagnostic/treatment data and develops nursing assessment. Reviews, assesses, measures, and evaluates wounds and wound progress/status, assigns grade/staging of wounds, and may photograph wounds.
Contacts clinician to get orders. Cleans and irrigates wounds, determines appropriate dressing, and applies or changes dressing as required; may assist with debridement and other wound treatments as requested. Interprets diagnostic data, determines when a change in care is appropriate and makes decisions regarding treatment changes within defined scope of responsibility. Prepares patient for physician/clinician visit.
Answers questions and provides health care education and guidance, in collaboration with interdisciplinary care team, to patients and/or caregivers in wound care management, including documenting patient education. Explains/interprets results, findings, and diagnoses to patient and/or patientâs family within standards of practice. Manages and coordinates patient case/wound management in accordance with established standards, strictly defined timelines, and overall plan of care. Identifies, communicates, tracks, and monitors all steps along the continuum of care and maintains plans of care.
Participates in rounds/examinations conducted by physician/clinician during clinic hours to develop/confirm patient plan of care, including observing/documenting wound care treatment and identifying next steps in accordance with the established clinical pathway and overall plan of care. Ensures that lab/diagnostic work is done and that appointments/referrals are scheduled as necessary, and follows up to ensure continuity of medical care. Maintains contact with patientâs clinician and advises other care providers of developments in patientâs condition when indicated. Identifies and documents post-discharge needs of assigned patients so that in-hospital, home care, and/or community resources are used appropriately.
Makes referrals to other healthcare providers or community resources as necessary. Confers directly with healthcare providers and resources regarding diagnosis or case management issues/problems. Keeps abreast of trends, developments and regulatory requirements impacting matters within designated scope of responsibility. Participates in community/staff education with respect to wound care.
Completes all required documentation in a timely, accurate, and thorough manner in accordance with FHS standards and procedures. Documents, in a timely, thorough, and accurate manner, nursing care and patient status in medical record in accordance with FHS procedures and standards of care. Ensures appropriate consent for treatment is obtained before proceeding with patient care. Accurately and timely completes necessary documentation for billing, including accurately applying codes and reviewing patient âsuper billsâ completed by clinicians to ensure they meet FHS standards and regulatory requirements.
Reviews and interprets clinical documentation and validates modifiers and assigned reimbursement codes to medical diagnosis, service level, and/or clinical procedure. Interacts with physicians and other clinicians/providers regarding billing and documentation policies and procedures. Initiates and monitors the Quality Management program for the Wound Care Center. Monitors the healing rate on assigned patient load in accordance with prescribed standards.
Accurately completes discharge documentation at the time of discharge. Selects patients who meet the Conservative Care criteria and completes appropriate paperwork. Cultural Sensitivity and Competence: Demonstrates proper use of communication tools/materials for effective communication and understands how the culture(s) of patient populations can affect communication, collaboration and the provision of care, treatment and services. Patient Population Served: Demonstrates knowledge and proper skills associated with the departmentâs defined specific populations served.Performs related duties as required QualificationsEducation/Experience: Associateâs degree in nursing, and two years of related work experience working with an interdisciplinary clinical team in one or more of the following clinical areas: Med/Surg, Gerontology, Home Health, or Wound Care that demonstrates attainment of the requisite job knowledge skills/abilities.
Bachelorâs degree in Nursing preferred License/Certification: Current Washington State Registered Nurse licensure required. Current healthcare provider BLS Certification required. Current professional certification (e.g. WCC, CWON, CWS or equivalent) is preferred.
