The individual selected for theRegistered Nurse (RN), Home Based Primary Care (HBPC)positionwill function under the supervision of the Nurse Manager (NM) and is accountable for coordinating patient care using evidence-based practice. TheRN's responsibilities will include, but are not limited to, planning, coordinating, and implementing daily nursing activities for a population of homebound, chronically-ill Veterans in Jefferson, Grant and Pulaski counties.HBPC is an interdisciplinary model of care that provides a blend of primary and palliative care to a largely geriatric population of Veterans. TheRN adheres to established policies, standards of care and standards of practice.The selected individual must demonstratethe ability to work effectively with other members of the team including the physician, advanced practice nurse (APN), rehabilitation therapist, social worker, clinical pharmacist, dietitian, psychologistand chaplain; demonstrate evidence of strong positive interpersonal/customer service relations and verbal/written communication skills; assist with maintaining effective evaluation programs on quality nursing care and personnel performance; foster participation in process improvement programs and clinical research; demonstrate sound decision-making, critical thinking, and organizational skills.
Relocation expenses will not be reimbursed.

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: Accreditation Commission for Education in Nursing (ACEN) formerly known as The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Must be proficient in spoken and written English.
Current certification in BLS.
Valid state driver's license.
Applicant's education and length of nursing practice (experience) will be considered by a Nurse Professional Standards Board in determining the grade and salary of the applicant selected. The recommended salary may be at any point in the range listed for this vacancy.
