Avamere Georgian House of Lakewood is hiring a full-time Admissions and Social Services Coordinator for this 73 bed Skilled Nursing Facility . The primary responsibility of this position is to obtain required information and admit residents in an efficient manner. The ideal candidate must be driven by and passionate for quality patient care and quality of life.

· Admit, transfer and discharge residents.
· Assist in interviewing residents, guardians and/or sponsors and obtaining required admission information and signatures.
· Obtain information from records as authorized/required for insurance companies, Medicare, etc.
· Maintain daily and monthly census reports, and assist the Nurse Managers in scheduling resident assessments.
· Assist in coordinating the social service programs of this facility.
· Assist in developing assessments and written plans of care to identify the needs of each resident.
· Effectively communicate information concerning a patient’s condition with other departments and government agencies.
· Participate in the discharge planning, providing information to residents and families of programs available.
· Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.
Qualifications and Skillsets:
· High School Diploma or equivalent.
· Must have at least 1 year medical records and patient admitting experience.
· Must have at least 1 year social work experience.
· Knowledgeable of current Medicare/Medicaid regulations governing admission and discharge requirements of healthcare facilities.
· Able to make independent decisions, and maintain a calm and levelheaded demeanor.
· Able to deal tactfully with personnel, residents, visitors and the general public.
· Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
· Knowledgeable in the use of computers, data input/retrieval/output functions, etc.
