The Touch Nurse Practitioner ensures effective and efficient treatment of our Touch members; providing BEST care to our patients, their families, our facility staff, our physicians, and each other. This individual will be responsible for managing patient care at multiple facilities through the implementation of cohesive and efficient processes, with emphasis to include patient and family satisfaction and physician and facility support. This individual provides general medical care and treatment to members in institutionalized settings such as nursing homes, assisted livings, or board & care facilities, in collaboration with the Physician. Participates in On-Call program which may include after-hours, weekend and holiday calls and visitations.
Responsible for managing patient care and treatment in collaboration with the physician. Primary duties may include, but are not limited to: Performs health assessments and preventative health measures within prescribed guidelines and physician instructions. Orders, interprets and evaluates diagnostic tests to indentify and assess patients clinical problems and health care needs. Discusses case with physician. Formulates and documents care plan. Prescribes medication or other forms of treatment
Requires an MS in Nursing with emphasis in Family, Adult, or Gerontological practice
; 1-2 years of experience working in long term care, SNFs, palliative care/hospice settings is highly preferred, but not required. Current unrestricted RN license and NP license in applicable state required. Must be willing to drive significantly within the assigned CareMore service area.
