Licensure required: Current Indiana R.N. licensure or eligible.

Experience required: At least three ye ars current clinical experience in a hospital setting. Actual experience in utilization review/discharge planning is preferred.

The primary responsibility of this position is coordinating all aspects of patient's hospitalization via activities related to clinical treatment, resource management, quality improvement, and medical disharge planning.

This PRN or "float" position works on an as needed basis, with no set schedule and no guarantee of hours. Employee benefits are not available for this position.

Some primary duties include Evaluate all admissions to assure patient meets acute care criteria.

Refer questionable cases to a Physician Advisor for review. Ensure collaborative planning processes are maximized and informed decisions are made; in conjunction with the patient, family and healthcare team assess and evaluate medical, rehabilitative, psychosocial and physical needs of all observation and inpatient admissions to ensure appropriate discharge plans are implemented. Internal and external resources will be utilized as appropriate to ensure patient needs are met after discharge. Reassessment and reevaluation of the planning process and patient goals are ongoing. Continual reassessment and reevaluation of the planning process is performed to assess the appropriateness for the plan of care, evaluate progress toward goals, and ensure that the plan is implemented in a timely fashion. Must attend multidisciplinary care rounds to assist in the coordination of patient care, discharge planning process and informing team members of current patient status and post discharge care planning process. C ollaborates with the physician and interdisciplinary healthcare network throughout the continuum of care. Reviews plan of care and expectations with the patient, family, physician, and healthcare team members. Identifies potential problems to treatment plan. These are identified through constant monitoring and re-evaluation of the patient’s progress toward the desires outcomes and goals. Be flexible in plan modifications. Determine the patient’s level of care and their readiness for discharge or transfer to the next appropriate level of care. Understands and utilizes knowledge of reimbursement and managed care in the decision making process. Communicate with outside agencies involved in the patient’s pre-hospital care as well as those planning for post hospital care. Provide necessary data to make a referral to the appropriate discharge agencies. Provide pertinent information to facilitate transition from acute hospitalization to post hospitalization care. Discuss the HIPPA regulation with the patient to make sure it is acceptable to discuss plan of care with family members. Acts as a patient and family advocate by assisting the patient in achieving autonomy and self determination to reach their goals. Help the express their views and choices by eliciting preferences and priorities from the patient and family in regards to the discharge planning process. Perform follow-up phone calls per criteria to the patient within a timely fashion post hospitalization. Stress to the patient the adherence to and the understanding of post-hospital treatment plan. Intervenes as needed to provide safe post hospital care. Phone outside referral agencies as appropriate. Must complete all paperwork and phone calls to insurance companies and other healthcare agencies by the end of the working day. Contact Third Party Payers with medical information in order to obtain certification/recertification.

Comply with all applicable policies and procedures, including but not limited to: all departmental and functional policy and procedure, maintaining acceptable attendance records, arriving for duty punctually and following dress code. Participate in performance improvement activities as requested. Assume information management responsibilities. Must have knowledge and skills pertinent to the functional approach to patient care and the organization. Perform duties as an active member of the healthcare team. Demonstrate a knowledge of team dynamics and contribute to team building and skills related to participation in and strengthening of the multidisciplinary team approach to the delivery of patient care and organization function.
