BAYADA Home Health Care is seeking an energetic, self-motivated Registered Nurse to fill the position of Nurse Clinical Liaison for our Gloucester County office. As Clinical Liaison, you will represent BAYADA Home Health and Hospice services at skilled nursing facilities, sub-acute rehab facilities and other community resources in Gloucester County, NJ.

The Clinical Liaison Nurse is a critical role that helps identify our current and future clients and facilitates the smooth transition to bring clients home. As Clinical Liaison, you will focus on building and maintaining relationships with clients and families, discharge planners, social workers and other health care and community personnel. In this role you will utilize your nursing abilities to assess patients' medical and social needs to identify appropriate home care services. Working with the office staff, you will lead the intake process and coordination of home care and hospice services for clients.

Responsibilities:
This position requires an individual who is able to develop and build strong, lasting relationships with referral sources, clients and families. Prior home care, hospice, case management or discharge planning experience is preferred. Excellent planning, organization and presentation skills are critical. A current New Jersey RN license with 2-5 years of recent nursing experience is required.

Current NJ RN license
Two (2) years experience in clinical practice with prior discharge planning, liaison or marketing experience preferred
Demonstrated customer service ability
Excellent organizational, interpersonal, telephone, documentation, and communication skills
Ability to travel to local facilities and physician offices as needed
