Reporting to Hospice/Homecare Program Manager, provides nursing care under RN supervision for patients in assigned clinical area.
graduate of accredited LPN program
1 - 3 years experience
LPN License- NH state or other Compact State
BCLS within 30 days of hire
Driver license - valid
Supplements and provides direct nursing care to patients and assists in the nursing process under the supervision of a registered nurse. Ensures patients have timely and accurate orders prior to delivering services.
Provides interventions, in accordance with the nursing plan of care, to meet patient outcomes. Documents outcomes of interventions.
Monitors/evaluates medical condition of patients and documents and reports changes to appropriate personnel and team members.
Collaborates with other health care team members to adjust the plan of care as necessary to ensure optimal outcomes. Updates plan of care if needed under supervision of registered nurse.
Provides patient/family education and counseling in learning self-care techniques, in accordance with agency standards.
Monitors and ensures accuracy of nursing documentation on patient medical record: routine notes, clinical and progress notes, medication reconciliation. Completes documentation same day as visit, per agency standard.
Ensures safe, timely and accurate medication administration per MD order.
Meets agency obligations for weekend, holiday and on-call coverage, and expected productivity standards.
