Statement of Purpose:
Supervises and coordinates patient admissions activities in either the long term care facilities, patient’s homes, hospitals, ALF’s & Hospice Houses by performing the following duties.

Primary tasks, duties, and responsibilities:
1.Promotes and practices the Hospice mission and values and follows policies and procedures of Tidewell Hospice.
2.Thoroughly explains Tidewell continuum of services to patients and families.
3.Secures proper signatures on all admission documents.
4.Completes the Suncoast medical record database utilizing the nursing process.
5.Collaborates with the admission office coordinators all pertinent information.
6.Reports to and utilizes voicemail system where appropriate to all Tidewell team members in order for patient care to begin.
7.Communicates with Director of Admissions; attends team meetings.

Secondary Responsibilities:
Every incidental duty connected with the RN Admissions position cannot be specified in the job description and the colleague, at the discretion of the Director of Admissions, may be required to perform duties that are not included in this job description.

Physical Requirements:
1.Ability to communicate and establish effective working relationships.
2.Ability to work, at times, under pressure.
3.Requires ability to manage multiple priorities and to be detail oriented.
4.Must be able to work independently and as part of a team.
5.Ability to travel within the four county area services.

Educational/Professional:
1.Must possess a clear and current Florida Licensure as RN, BSN o MSN preferred.
2.Minimum one year experience as a Hospice RN preferred.
3.An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.

Knowledge, Skills and Abilities Required:
1.Ability to asses situations, consider alternatives and apply reasoning and judgment for appropriate course of action.
2.Prioritizes and uses time effectively to complete assigned tasks.
3.Demonstrates knowledge of Hospice philosophy, federal and state regulations, conditions of participation, policies and procedures.
4.Respects and maintains confidentiality of information obtained in job performance.

Machines/Equipment, Tools Used:
1.Knowledge of Suncoast database & Microsoft office products.
2.Ability to drive a car.
