The Social Worker/Case Manager is responsible for the coordination of Rehabilitation Services within the interdisciplinary team. Acts as an active member providing, coordinating and directing care specific to rehab services, referral management, discharge planning and follow up care.
ESSENTIAL FUNCTIONS:
1.
Assists in the orientation of the Rehab patient to his/her individual program.
2.
Assumes responsibility for making sure that the individual rehab plan is implemented.
