The Clinical Dietitian, Diabetes Specialist, will perform focused nutrition assessments, recognize and prioritize complex data in order to identify needs of patients with diabetes across the life span and provide therapeutic problem solving, counseling and regimen adjustment to assure appropriate nutrition intervention and adherence to regulatory standards (IDPH, JCAHO, etc.). Monitors and evaluates interventions, makes adjustments to plans, as needed, and reports results of quality outcomes. Oversees the planning, implementation, and evaluation of the DSME entity at all sites. This position also is responsible for coordination of the diabetes education program.

Position Requirements:Minimum Education:
Required: Bachelors Degree
Preferred: Masters Degree

Minimum Experience:
Required: 3-5 years of previous job-related experience
Preferred: 6-10 years of previous job-related experience
Minimum Experience Details: Has academic preparation and/or experiential preparation in program management and the care of persons with a chronic disease.

Required: Registered Dietician,Specialty Registered Dietician

Position Responsibilities: Use principles of teaching and learning theory and lifestyle counseling to help clients confidently and effectively manage their disease. Coordinates and documents quaility improvement projects and planning and review of patient education programs. Provide leadership on nutrition issues within and outside the profession through initiatives such as presentations or publications for the organization and/or community. Performs other duties as assigned.
